Audition Application and Payment

You must schedule your audition before submitting this form. 

Musicians who are auditioning on more than one instrument or for multiple ensembles must submit an audition application form for each instrument/audition.

Registration is not complete until 1) this form is completed and submitted and 2) initial payment is received.
